About the Speakers:

Dr. Jonathan Flacker: Regional Chief Clinical Officer; Doctor Jonathan Flacker specializes in internal medicine and geriatric medicine. He is a member of the American College of Physicians and the American Geriatrics Society. He enjoys working with the senior population the most and his favorite part of being a doctor is learning about the patients he cares for.

Dr. Stefanny Santana: Primary Care Physician; During medical school, Dr. Stefanny Santana Rivera “learned the importance of primary care, prevention, and social determinants of health,” which led her to realize her “passion for prevention and counseling the elderly population.” Developing relationships with her patients and their families is her favorite part of being a physician. “I am a person who loves to learn and loves to share knowledge with people,” she says. She is “open and honest” with her patients and likes serving as a guide to help them “improve their overall health and wellbeing.”

Dr. Harold L. Rahming: Center Clinical Director; At an early age, Dr. Harold Rahming knew he wanted to serve as an “agent of change” for the health of his community. “Health care inequity and social determinants play a major role in the overall health of our society,” he explains. Dr. Rahming is “deeply grateful” to be a primary care physician and takes pride in serving as an ally for his patients. “They have full autonomy to make their own decisions, and we are only here to guide them,” he says.
